Parental Consent and Acknowledgement Form

The Parental Consent and Acknowledgement Form must be on file for all Trinity Preschool families before the start of each school year. You only need to submit one form for all children you have enrolled at Trinity – simply list each child at the top of the form. This form is due by August 1st.

Trinity Preschool Policies
Please read all policies below and check each box in agreement and consent for each.
Medical Consent*
I consent to Trinity Preschool's medical policy.
in the event of a Medical Emergency (as defined below), the undersigned, being the lawful parent and/or guardian of the above named child(ren), hereby authorizes the Trinity Preschool of Berwyn and any of its staff or employees to provide for, approve, and authorize any health care at any hospital, emergency room, doctor's office, or other institution; employ any physicians, dentists, nurses, or other person whose services may be needed for such health or dental care to the child. "Medical Emergency" shall mean an injury, disease, or any other medical condition affecting the child that may require immediate treatment or for which Trinity has unsuccessfully attempted, after reasonable efforts, to contact the parent and/or guardian on record before administering or authorizing any treatment.
First Aid Consent*
I consent to Trinity Preschool's first aid policy.
The parent or guardian consents to first aid treatment by Trinity staff in the event of a minor injury.
Class Trip Consent*
I consent to Trinity Preschool's class trip policy.
The parent or guardian consents to the participation by the child in class trips and walks approved by Trinity and to the participation of the child in all events relating to such activity.
Photo Use Consent*
I consent to Trinity Preschool's photo use policy.
The parent or guardian consents to the use of photographs of the child by Trinity for educational, informational, and promotional purposes, including, but not limited to, the posting of photographs on the Trinity website, Trinity's social media site(s), brochures, and in-house materials.
Parent Handbook Acknowledgement*
I acknowledge that I have been provided a copy of the Trinity Preschool Parent Handbook and agree to abide by the policies and procedures outlined in the Handbook.
The parent or guardian understands and agrees that it is their responsibility to read and familiarize themselves with policies and procedures outlined in the Parent Handbook, and agrees to abide by them. Information contained in the Parent Handbook may be subject to change.
Distance Learning Consent*
I consent to Trinity Preschool's distance learning policy.
The parent or guardian consents to the participation by the child for the purpose of distance learning in the case of an extended school closure.
Limitation of Liability*
I consent to Trinity Preschool's Limitation of Liability policy.
By submitting this electronic form, the parent or guardian assumes all risks associated with the consents granted in this agreement and agrees to hold harmless, release, and indemnify Trinity Preschool from all liability, claims, demands, damages, costs, expenses, actions, and causes of action arising or to arise by reason of the consents granted in this agreement. Submission of this form serves as the electronic signature of the parent or guardian named on this form.
